What are the parameters to be specified- The Server name and the
Server Path. I tried giving the computer name and the reportserver path. It
doesn't show me any errrors but I'm not able to resolve the action and
execute it.
Can anybody please give me detailed steps of creating and executing
the report action as I am not able to find out where I am going wrong.

I presume you talk about an URL action?
the MDX syntax should be:
"http://server/reportserver?/Folder/Reportname&Year=" +
Time.Currentmember.Name
