Whatever may be causing your data duplication problem, one thing I'm sure: it
has nothing to do with the fact that the SQL Server instance is clustered.
Personally, I'd start by looking at the app.

Notice that the duplicate entries all happened 1 minute after the original
insert.

Could this behavior be caused by a miss configuration or problem with the
SQL Server cluster?

If so is upgrading to Service Pack 2 likely to resolve the issue or is there
some other solution that is recommendable?
