A failover is exactly the same as an unexpected SQL server stop and restart
as far as the client can tell. So, all connections are lost for a brief
time, any transactions are rolled forward or back depending on their commit
status. The client needs to detect the connection failure, reconnect the
SQL, and resubmit any failed transactions.

When an instance fails over in a cluster from Server A to Server B, does
the client detect the outage or lose any information if it's in the middle
of a transaction? Or does SQL Server remember the transaction, rollback the
database, fail over the instance and restart the transaction? Thanks.

No, it has to be coded by the application developers.
