I have a sucessfully running cluster with instance IDs available on both, and
with failovers sucessful. I added a third server, Windows 2008 SP2, and have
added another instance of SQL 2008. My problem is that the only one of the
first two SQL instance IDs are available within SQL on the new server, and
the new instance ID is not available on the original two servers.

SQL 2008 installs on a per-node basis. You have to add each node to each
instance running the installer locally on each node.

There is no more remote installer that adds all nodes at once.
