I suspect this is a problem either with the HBA or the HBA drivers,
just try to sort out the issue with SAN engineer or open-up a case
with HP.

Hi again,
from what you have posted it seems you lost the disk unit
holding the Quorum resource database.

Once you loose the Quorum, the cluster service
can't run anymore until you fix it (which is
a documented procedure in the Knowledge Base).

The serious problem, however, is not that you have
lost the Quorum, but why you lost it.

Looking at the log you posted, it seems that the disk
unit can't be correctly presented to the server.

This kind of problems are often related to:

- HBA configuration (configuration of paths)
- a buggy HBA device driver
- a buggy HBA device

Of course these are only guesses, you will need to open
a case and work with the HP technical contact that installled
you the SAN to sort it out.
