So I apply SP3 then Hotpatch 4? Do I need to apply SP1 before SP3?

SP3 then Hotpatch 4 : Yes

SP1 before SP3: The notes on the download page, says: Service Pack 3
contains all fixes included in previous service packs and hot patches.
