Default Quick Access Toolbar only showing in Design View - 07-10-2012 , 08:04 PM

I'm using Access 2010.
The QAT is displayed normally in design view, but when I select form view,
it reduces to the down arrow only, with all options grayed out.
If I open Options, QAT there is nothing to select in the 'Choose Commands
From' list box.
Under Options - Current Database - Ribbon & Toolbar Options, I have nothing
selected for Ribbon Name, Allow Full Menus and Allow Default Shortcut Menus
checked.
I'm guessing that I'm missing something pretty elementary here, but could
some one tell me what it is?

I don't know Bob! I just made a QAT (to see if I could help you) and
I am getting the same results!

<ribbon startFromScratch="true">

<qat>
<documentControls>
<button idMso ="Copy"/>
<button idMso ="Paste"/>
<button idMso ="PrintDialogAccess"/>
<button idMso ="FileCloseDatabase"/>
</documentControls>
</qat>

The same thing is happening when running the Ribbon Example 8 (QAT)
from AccessRibbons (http://www.accessribbon.de/en/?Downloads:8)

I don't think this helps any:
