[Info-ingres] E_LC0001 - Name Server ID Incorrect

OS: SCO OSR5.0.5
Version: II 2.0/0001 (sos.us5/00)
Patch: 6900

We were unable to connect to the database this morning with following
sequence of errors:

GCA Protocol service (GCA_REQUEST) failure.
Internal Service Status E_GC0025 -- Unable to connect to Name Server:
Name Server ID incorrect (E_LC0001)

Failed to connect to DBMS session. (E_LQ0001)

Error connecting to database 'dbname'. (E_UG000F)

There is a lot of info in the logs (iircp.log) that are not normally there
but nothing stands out.

What cause this error?
How do I recover from it?

OS shutdown forced ingres termination before ingres could complete orderly
shutdown, rendering iidbdb inconsistent.

Did a rollforwarddb on iidbdb from latest checkpoint.
