In the old days what you saw on a Mac's screen "100%" was actually what
came out of the printer (more or less), but thesses days screens are no
longer all 72dpi and what you see on-screen is usually slightly smaller
than what gets printed, even when viewed at "100%".
Preview mode MAY give you a slightly better idea of what will come out,
but even that's not guaranteed, and neither is the PDF preview in Mac
OS X.
The problem with the fields and the fonts may partly also be caused by
differences between the operating system's on-screen font and the print
font (the laser printer may have a slightly different version of the
font) and or a by-product of the screen being 72dpi-100dpi while the
printer has a higher resolution.
"Rough edges and skips"!?! That sounds a little like you've been
pressing the Return key and/or using spaces to align the edges of text
- two things you shouldn't really do on a computer (there's an old
desktop publishing saying about "the Mac is not a typewriter".)
Bascially you have to go through a trial-and-erro process until the
printout meets your needs. You may even want to have two separate
layouts - one for on-screen and one for printing.
