Your "problem with pages after page 1" issue sounds to me like a
Layout Part problem. Check the layout to make sure there aren't any
Leading Grand Summaries or the like.

There was no Leading Grand Summary but there was a Leading Sub Summary
that I took
out. This did not correct the problem but did show another symptom

Page 1 is fine. Page 2 is about 1/4" to the right. Page 3 is about
1/2" to the right. Page 4 is the same as page 2.

Problem went completely away when I unchecked the Facing Pages
checkbox
in the Printing tab of the Layout Setup.
