patient data

Does any one have a simple template that would allow me to enter
patient ID number, a few drop down fields (e.g. tobacco use: yes/no),
and longer fields for typing, e.g. assessment/plan. Not looking for a
full blown electronic medical record, rather, a database of patients
I've seen from which I can print a report.
